Privacy Policy
Introduction: This document describes the Formula II privacy policy regarding information that is collected on this site for the purposes of program administration. We only collect personally identifiable information that is voluntarily provided by the visitor to our site. This Privacy Policy is intended to provide all of the information necessary for a complete understanding of how we handle the personal information collected.
Personally Identifiable Information Collection: Personally identifiable information collection is required to register on this site. The information that we collect may include name, email address, and mailing address. Other information such as phone number may be requested in order to participate in special programs such as promotions or surveys but is optional. By supplying your personally identifiable information you are consenting to our use of that information in accordance with this Privacy Policy and the Terms and Conditions.
How Personally Identifiable Information is Used: The information we collect is used to contact you with account information and/or updates, to announce special offers or promotions and to inform you if you have won a sweepstakes or contest. In addition, if you have chosen to opt-in you may receive additional communication such as newsletters that may be of interest to you.
Use and Disclosure of Personally Identifiable Information: Personally identifiable information is never shared with a third party unless it is required for the completion of a transaction (for example a fulfillment house shipping a reward to you) or other service that may be required to operate our site. We will share aggregate data with partners but that will be designed so that the individual is not specifically identified.
Non-Personally Identifiable Information Collection: non-personal information on all visitors to our site is collected through the use of technology. This information includes IP addresses, browser type, number and frequency of visits, and time of access. This information is used for statistical purposes relative to site activity and performance only.
Refer-a-Friend Information: there may be times when we offer an option for you to share program or promotional information with a friend. In order to fulfill this request, we will ask for your friend’s name and email address. This information will only be used to communicate with your friend via email regarding the program or promotion you specify. Your friend will not be subscribed to any subsequent communication from us unless they visit the site and complete the registration process.
Cookies and browser information: Formula II will use cookies so that participants must have a browser that is set up to accept them. Formula II will also retrieve information from the participants’ browser including: type (e.g., Netscape), platform (e.g., Windows), date and time of visit, site activities, account information, and transaction details (e.g., number of points accrued). This information is used for site tracking purposes.
Users may also be asked to download specific software such as Active X controls to enhance system performance.
Opt-in Email Communications and Email: Formula II will not send unsolicited promotional email. Program participants will need to sign-up to receive information and promotions targeted at their interests. Members may opt-out of any email subjects they have signed-up for at any time. If you have registered with us, you may continue to receive customer service emails when necessary, even if you elect not receive promotional emails from us.
Sweepstakes and other Promotions: program members will be allowed to opt-in to participate in online sweepstakes and promotions. If you opt-in to participate in these activities, personally identifiable information will be required to notify winners and award prizes. Information requested usually includes name and shipping address and demographic information (e.g., gender).
Surveys: program members will also be allowed to opt-in to participate in surveys. Personally identifiable information may also be required to complete a specific survey and typically includes demographic information such as age and gender.
Access to Personally Identifiable Information: If you have created a Formula II Rewards Program Account with us, you can review, update or correct the personally identifiable information you previously submitted to us by logging in with your user name and password. You cannot however, delete your personal information from our system without contacting customer service and asking for it to be removed. Please note that it may take up to seven days for your request to be processed. You may continue to receive customer service emails when necessary, even if you elect not to receive promotional emails from us. Please click on “Forgot your password” on the navigation bar to help you recover your password if you have forgotten it.
Formula II Partners and 3rd Party Websites: Formula II will provide links to other websites. Formula II will make every effort to ensure that these sites have strict Privacy Policies to protect members. The Privacy Policies of these sites are not governed by the Formula II Privacy Policy.
Formula II will not share personal information with third parties unless instructed to do so by the member.
Formula II will have occasion to hire outside companies to assist with services on our behalf (e.g., sweepstakes). These companies will only be provided with the data necessary to deliver their service.
Formula II may share non-specific information in aggregate form to its partners or other third parties. Information we gather may also be sent to a third party for the purpose of organizing and analyzing the data.
Security: Formula II uses appropriate security such as firewall security and encryption technology to protect member information.
Changes to the Policy: changes to the policy will be posted on the Privacy Policy website.
Changes to the Company: registered user information may be transferred in the event that the company is sold, merges with another company and/or sells a portion of its assets. All registered users will be notified of any such changes either through email or through our Web site.

Terms of Participation: individuals must be 18 years of age or older to participate in programs on this site.
Formula II allows only one email address per registered user.
Limitation of Liabilities: Formula II is not liable for any damages whether direct or indirect relating to site program participation.
Laws: any dispute or claim would be resolved in the State of Pennsylvania.
